checks payable to: Church in San Gabriel) by the assigned date. The registration form is the same sheet as the Emergency/Medical
Authorization Form and everyone (in HS & JH) is using the same form.
There are two (2) forms (Medical
Release Form & Rules and Regulations) that you must read & fill out completely (Don't
forget to sign at the bottom of Rules and Regulations form). To print forms
